Most dental software tries to replace everything. Dentra does the opposite — it ships a narrow operational wedge on top of whatever the clinic already uses: recall recovery and treatment-plan acceptance. Two high-value workflows, designed deeply, with multi-clinic workspaces, audit logs, and AI-agent-aware development rules baked in.

Wedge over breadth

Rejected the urge to replace a whole practice-management system. Chose two high-value workflows with measurable revenue impact, then designed around them.

Multi-workspace data model

Workspace → clinics → users → patients → cases. Role-based permissions, audit logs, subscription gating — designed for real SaaS from day one.

Monorepo with worker tier

Separate web, admin, api, and worker apps for background jobs. Shared packages for ui, types, database, env validation.

AI-agent rules as architecture

AGENTS.md, CLAUDE.md, .cursor/rules/, llms.txt, ai/HANDOFF.md — explicit rules that keep AI coding assistants aligned with the architecture instead of inventing it.

Documentation-first product

PRD, architecture doc, database plan, API contracts, risk register, release checklist — written before half the code, so the build stays on track.
